Position Title: CDP Intern Program: Clinical Day

Program Reports to (Position): Facility Director

Location: Woodrow St. Columbia, SC

Job Summary:

The Intern, under the direction and supervision of the program LCS, is responsible for mastering the responsibilities as assigned. The intern must be available during program hours 7am3pm. Intern is responsible for a maximum of five clients under the direct supervision of LCS and Facility Director.

Primary Duties and Responsibilities:

1. Maintain strict adherence to Medicaid, State, COA, standards and agency guidelines, policies and procedures.

2. Understands and adheres to code of ethics and practice for functional area in alignment with AWS mission, values, goals and directives in all work performed.

3. Maintains confidentiality of sensitive information.

4. Observe and engage in IEP meetings for CDP clients.

5. Observe and engage in initial orientation with new clients

6. Maintain involvement in assigned client’s care and treatment, including assessment, provision of direct treatment services, and evaluation of client progress.

7. Maintain documentation according to Medicaid and COA standards and within appropriate timeframes.

8. Maintain caseload of at least three clients and be responsible for all documentation affiliated with assigned clients while under close supervision of LCS.

9. Assist and master the development of an initial IPOC within 45 days of service and Progress Review for every 90 days of service on each client.

10. Master and complete Clinical Summary Notes for each client when a Rehabilitative Behavioral Health Service (RBHS) is performed and is filed in the client’s clinical record within 10 days of completion.

11. Master and assist with preparation Monthly Progress Summary Note that summarizes the client’s overall status and progress in the program.

12. Provide individual counseling services to assigned clients a minimum of once per week and group therapy daily in accordance with authorized RBHS standards and internship schedule.

13. Ensure that services are provided to clients in a safe, efficient manner in accordance with accepted standards of clinical practice.

14. Participate in all assigned staff meetings as well as staff development activities.

15. Support agency activities.

16. Perform other duties as necessary

Minimum Qualifications:

Knowledge Of:

• Child and Adolescent Development/Behavior issues

• General knowledge of Social Work/Counseling Code of Ethics

• Must be able to pass SLED and Central Registry background checks

Skills In:

• Communicating effectively orally and in writing

• Organizational skills and ability to prioritize and complete tasks within the specified time frame.

• Establishing and maintaining professional relationships with clients, client family, staff, and community agents

Education and Experience:

• Pursuing a Master’s Degree in Counseling, Social Work, or related field.

• Students pursuing Bachelor’s Degree will be considered on a case-by-case basis evaluating the individual’s work experience and ability to work with the program population.

Program: Columbia Youth Home Program

Job Summary:

The Intern, is responsible for mastering the responsibilities as assigned. The intern must be available during program hours 9am-10pm (to be determined by Facility Director and Intern). Intern is responsible for a caseload of no more than two clients under the direct supervision of the HSP/CC and Facility Director.

Primary Duties and Responsibilities:

1. Maintain strict adherence to all State, FPB standards, COA standards, DJJ standards, DSS standards and agency guidelines, policies and procedures.

2. Understands and adheres to code of ethics and practice for functional area in alignment with AWS mission, values, goals and directives in all work performed.

3. Maintains confidentiality of sensitive information.

4. Collaborate with treatment team members via telephone, face to face and email

5. Complete clerical duties (i.e., filing, maintaining charts, maintain multiple databases)

6. Participate in treatment staffings for clients.

7. Master and participate in assessments.

8. Maintain documentation according to DSS, DJJ and COA standards and within appropriate timeframes.

9. Maintain caseload of no more than two clients and be responsible for all documentation affiliated with assigned clients while under close supervision of HSP/CC.

10. Facilitate individual and group counseling.

11. Assist and master the development of an initial IPOC within 30 days of service and Progress Review for every 90 days of service on each client.

12. Master and assist with preparation of Progress Summary Note that summarizes the client’s overall status and progress in the program.

13. Ensure that services are provided to clients in a safe, efficient manner in accordance with accepted standards of clinical practice.

Intern is responsible for a caseload of at least four clients under the direct supervision of the Facility Director.

1. Maintain strict adherence to Veteran Administration, COA and AWS standards and agency guidelines, policies and procedures.

2. Understands and adheres to code of ethics and practice for functional area in alignment with AWS mission, values, goals and directives in all work performed.

3. Maintains confidentiality of sensitive information.

4. Observe and engage in initial orientation with new clients.

5. Maintain caseload of at least four clients and be responsible for all documentation affiliated with the client.

6. Facilitate weekly group empowering sessions.

7. Perform Psycho-social assessments on a weekly basis.

8. Ensure that services are provided to clients in a safe, efficient manner in accordance with accepted standards of clinical practice.

9. Participate in all assigned staff meetings as well as staff development activities.

Position Title: SSVF Intern

Job Summary:

The Intern, under the direction and supervision of the SSVF Lead Case Manager, is responsible for mastering the responsibilities as assigned. The intern must be available during program hours, 8:30am – 5:00pm.

Primary Duties and Responsibilities:

1. Provide administrative support to the SSVF case managers and other staff (respond to program inquiries, assist with program documentation, phone calls, clerical, etc.)

2. Coordinate and conduct individual participant needs assessments, prioritizing services based on screening tool with assistance of the program participant

3. Provide crisis intervention as needed to stabilize participant (food, utilities, shelter, health services, etc.)

4. Facilitate development of Individual Service Plans to include goals shared by both CM and veteran to sustain housing stability

5. Document progress and other services provided in client files.

6. Assist case managers with basic participant follow-up services, including participant updates with other service providers, checking progress with benefit applications (online and via phone with VA and Benefit Bank), check-ins with participants, etc.

7. Identify potential affordable housing opportunities of varying types

8. Coordinate and establish referral source network, attend community meetings, and conduct outreach activities as needed.

9. Ensure client confidentiality in accordance with established procedures and regulations 10.

10. Perform all other duties as assigned.

Minimum Qualifications:

Knowledge Of:

• Veteran’s specific social, behavioral, and mental health issues

• Low income and homeless population specific social, behavioral, and mental health issues

• General knowledge of Social Work/Counseling Code of Ethics
